Dear Parents and Guardians,

Please use this information as a guide to understanding your child’s Community Service Verification Record Form.

§ Community Service hours must be completed after school hours unless special permission is granted prior to the activity. 

§ The IB Coordinator should be the only people signing the verification record form. The IB Coordinator will determine how many hours of community service will be granted for each activity (60 minutes = 1 hour).

§ Students are to bring in documentation of the community service activity if the activity occurred outside of Salem with another agency. The IB Coordinator will not sign the verification form if students do not have this documentation. A signature is needed by a parent/guardian and/or the individual running the activity.

§ Extracurricular activities such as Boy Scouts, Girl Scouts or church groups do not count towards community service hours, however volunteering opportunities associated with those activities will count.

Community Service Requirement:

6^th Grade: 10 hours

7^th Grade: 15 hours

8^th Grade: 20 hours
